This is an excellent video for demonstrating human falibility and how a lack of teamwork can lead to serious events. The investigation focuses on the conditions beyond "pilot error" that influenced the decisions made that led to the crash.

This DVD is full of examples of human errors. Good training tool. Well produced recreation with documentary interspersed at the correct times. Impactful.

An excellent example in the failure of "teamwork" in the fight deck. This airing clearly showed how the fear of questioning a superior (by a 1st officer and flight engineer) was one of several factors leading to such a tragic accident. Although airlines today encourage "teamwork" in the cockpit, it is still alarming that after 30+ years, such an occurrence can still take place, despite the 21st century technology that exists.
